What can we do to improve patient safety?Errors in health care in the USA alone cause between 44,000 and 98,000 deaths every year - more than car accidents, breast cancer or AIDS cause. (Kohn LT et al. To err is human: building a safer health system. 2000; 26.) We have the responsibility to improve this situation. In mechanical ventilation, it is not only human error that leads to serious patient injury – ventilation itself is an ever-present danger: Ventilator-induced lung injury and VAP are two of the key issues. In addition, health care providers experience increasing pressure to reduce costs, which can be at odds with their attempts to provide high-quality care.
