There is still uncertainty about the effects of higher PEEP and recruitment maneuvers (RMs) in ARDS patients. However, we do know that these patients are not the same and therefore clinicians should consider ARDS phenotypes when setting the ventilator. In this review, we look at the LIVE study from France (Constantin JM, Jabaudon M, Lefrant JY, et al. Personalised mechanical ventilation tailored to lung morphology versus low positive end-expiratory pressure for patients with acute respiratory distress syndrome in France (the LIVE study): a multicentre, single-blind, randomised controlled trial. Lancet Respir Med. 2019;7(10):870-880. doi:10.1016/S2213-2600(19)30138-91​), which was the first of its kind to investigate this question. This multicenter, randomized controlled trial compared a ventilation strategy personalized to the patient’s lung morphology with the standard of care in terms of outcomes in ARDS patients.
