Why is the flow shape different for the second breath? The patient is ventilated in pressure-support mode with no backup respiratory rate (RR).

Answer: Auto-triggering. This is a mechanical breath with no inspiratory effort made by the patient. On the full screen showing esophageal pressure (below), auto-triggering can be seen from the lack of patient inspiratory effort on the Pes waveform. On the pressure and flow waveforms, auto-triggering is characterized by the absence of a pressure decrease just before the mechanical breath, a reduction in maximum inspiratory flow, and a different slope for the inspiratory flow.
